Summer camp requirements

To attend summer camp, Dynamite and/or the State of Maryland require the following:

  1. Registration (follow steps below)

  2. Health Forms (please scroll down for button to online forms)

  3. Immunization Form (if camper attends school outside of Maryland (even if they live in Maryland), provided by your physician)

  4. Additional Forms (if camper requires emergency medication or has other medical requirements)

The State of Maryland required forms MUST be completed before your camper will be permitted in the gym. If your child has special medication needs, resides outside of Maryland, or resides in Maryland but attends school outside of Maryland, additional forms are necessary.

PLEASE KEEP IN MIND THAT WE WILL NOT BE SERVING FOOD THIS YEAR. IF YOUR STUDENT IS ENROLLED IN FULL DAY CAMP, THEY MAY BRING A LUNCH WITH THEM, BUT PLEASE REMEMBER THAT BOLT IS A NUT-FREE FACILITY.

CAMPERS WILL NOT BE PERMITTED IN THE GYM WITHOUT THE REQUIRED FORMS AS THIS VIOLATES MARYLAND STATE LAW.


Camp registration process

We know signing up for summer camps can be a hassle, so we've made it easier! Here are the steps:

  1. Click on the PARENT PORTAL link below and create an account on our parent portal/add student(s)

  2. Click on “Booking”

  3. Click on “Summer Camp”

  4. Select the camp type and camp weeks you are requesting. PLEASE NOTE: This year camp enrollments are not automatically approved, if you are signing up within a few days of your camp start date it may be declined. It may also be declined due to our limited space. Please only come to camp if your camp enrollment has been approved.

  5. Add payment info in parent portal and pay.

  6. Click on HEALTH FORMS link below (these forms are REQUIRED for participation)

  7. See ADDITIONAL FORMS section below if your camper attends school outside of Maryland and/or has allergies or other medical requirements.

  8. All forms MUST be processed before campers are permitted in the gym. Registration and Health Forms can be completed online; Additional Forms can be emailed to hello@boltparkour.com


HEALTH FORMS

The State of Maryland requires additional forms, which can be completed via the link below. These forms MUST be completed before your camper will be permitted in the gym. If your child has special medication needs, resides outside of Maryland, or resides in Maryland but attends school outside of Maryland, additional forms are necessary (scroll to the bottom of this page). 

CAMPERS WILL NOT BE PERMITTED IN THE GYM WITHOUT THE REQUIRED FORMS AS THIS VIOLATES MARYLAND STATE LAW.


ADDITIONAL FORMS

The following forms are also required:

  1. If your child attends school outside of Maryland, even if they are a Maryland resident, please submit:

    • Your child's Health Immunization Record

  2. If your child will self-administer medication or requires emergency medication during camp, please submit:

    • Medication Adminstration Authorization Form

  3. If your child has special health needs, please complete and submit:

    • Section 504
